Role Specifics and Benefits: As a Medical Social Worker on this travel assignment, you will be at the forefront of psychosocial assessment, care planning, and advocacy. Key responsibilities include performing biopsychosocial evaluations to determine patient and family needs; coordinating with medical teams, discharge planners, and community resources to arrange post-acute care, home health services, housing, transportation, and financial counseling; identifying barriers to treatment adherence and developing creative strategies to overcome them; delivering crisis intervention and counseling support to patients and families facing acute or prolonged illness; facilitating palliative and end-of-life planning where appropriate; documenting interactions accurately and in compliance with regulatory standards; and contributing to program development by sharing insights from your on-the-ground experience.
